Utilizes clinical judgment, independent analysis, critical thinking skills and detailed knowledge of case management program, medical policies and clinical guidelines to identify, review, assess and allocate patients for program participation according to their identified needs.
Collaborates with the clinical staff, physicians and other members of the patients healthcare team, to develop an individualized and comprehensive plan of care according to the applicable program specifications ensuring the coordination and integration of services as planned in the pertinent program revising the pan as needed to ensure high quality continuity of services and cost effective service delivery.
Utilizing the telephone and other technical and electronic communication formats, remotely monitors the care of the patient according to the program requirements. There may be situations and programs which require the case manager to meet the patient, the patients family and/or the physician face to face in the home, clinic or medical office setting.
Proactively contacts the patient on a frequent basis as specified in the program details to assess their situation and to provide assistance and answe
RN required with active unrestricted nursing licensure in practicing state.
BSN preferred 2 years of Nephrology Nursing (preferred) or Case Management required
Experience in a role related to patient education and/or case management is preferred.
Must exhibit management / leadership skills and be able to work with and influence staff in the clinics.
Must have knowledge of teaching-learning process, principles, and methods of adult education.
Understanding of renal and diabetes disease process and current management practices.
Excellent written and verbal communication is necessary.
Excellent and professional telephone skills.
Ability to function as a part of an integrated team.
Adequate presentation skills needed to prepare and deliver presentations to small groups telephonically or in person.
Ability to develop positive relationships with patients, families, physicians, staff and other disciplines.
Must be self-motivated, organized and able to plan a diversity of activities, and work independently.
Able to follow oral and written communication and communicate information to individuals or groups with varying levels of education.
Must exhibit practical computer skills
