HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
Heartland Hospice Services of Portsmouth Ohio has an immediate need for a Part-time Bereavement Coordinator (3 days a week/24 hrs).
Responsible to develop, implement and supervise the hospice bereavement program. Provides bereavement support through grief support, memorial services, family assessment and conference. Responsible for consultation to interdisciplinary team, client and family in issues related to grief and loss. - Consults with the Director of Volunteers regarding assignment and supervision of the bereavement volunteers. Participates in orientation, training and evaluation of bereavement volunteers. - Develops programs for bereavement group activities. - Develops a resource library of written material, videos, and referral sources for families and the interdisciplinary team. - Experience in family therapy regarding issues of loss, grief, death and dying. - Experience in developing and conducting programs of support. - One year experience in home health care or hospice preferred. Job Specific Details: Previous Hospice experience preferred.
Baccalaureate degree in counseling, theology or related field, or documented training in Grief and Bereavement.
