Assessment Coordinator
Inglis Foundation works with people with physical disabilities to create and provide practical solutions so they may pursue their goals. Inglis House, which provides long-term, residential care for nearly 300 adults with physical disabilities, is seeking an experienced, full time Assessment Coordinator.
Responsibilities include:
• Completes MDS for OBRA and PPS schedule on assigned unit to ensure accuracy, comprehensiveness and timeliness
• Maintains up-to-date knowledge and database on MDS and Care Plan regulations
• Coordinates Plan of Care meeting and MDS process for plan of care team, residents and families
Qualifications:
RN & LPN licensure in Pennsylvania, BSN preferred
Minimum 3-5 years RNAC experience
RNAC certification or actively pursuing
Knowledgeable in PPS scheduling, RUG optimization and Pennsylvania CMI
Superior verbal, leadership and organizational skills
Excellent computer skills a must
