Population Health Management Success Manager

Job Description
To support the service and administration of Meritain Health's Population Health Management products to achieve profitable growth. This will be accomplished through the development of strong external and internal relationships to achieve customer satisfaction, revenue, membership and earnings objectives for existing and prospective customers in assigned business segments, and products.

Fundamental Components
  • The PHM Success Manger role supports Meritain Health's Population Health Management Strategy.
  • The Success Manager provides program oversight and consultative support for clients in maximizing their Population Health programs while achieving program participation and budgetary goals.
  • In addition, the PHM Success Manager is responsible for developing and maintaining mutually rewarding working relationship with client contacts, internal team members and vendor partners to ensure renewal of case and retention.

Background Experience
  • 1-3 years of industry experience.
  • Bachelor's degree or equivalent work experience.
  • Requires strong communication, customer service focused and client account management skills

Additional Job Information
Demonstrated ability to analyze engagement and performance across multiple population health management programs. Ability to consult strategically with HR leaders, producer partners and key vendors. Strong understanding of population health programs and the impacts on health and wellbeing. Excellent project management, organization and presentation skills. Strong team player, able to handle multiple assignments and responsibilities simultaneously. Will be responsible as the PHM account contact for the performance of key population health programs for our customers, including implementation, analytics and report delivery, program recommendation, communications effectiveness and identification of cross-over sales opportunities.
