You'll enjoy the flexibility to telecommute* from the state of Arizona, as you take on some tough challenges.

Primary Responsibilities:
  • Provides initial telephonic outreach to identified high risk members for the purpose of providing a comprehensive assessment and care planning
  • Works closely with all medical management teams to engage patients in appropriate BH services and resources and assist with coordination of care
  • Promotes patient engagement in therapy, engagement with psychiatric appointments, compliance with the healthcare treatment plans, involvement and engagement with wellness services and engagement with primary care
  • Completes all tracking and reporting requirements for outcomes and evaluation
  • Maintains appropriate professional standards and provides appropriate follow-up for patients
  • Provides self-help recovery services and other wellness services
  • Provides connections to community-based supports and services related to social determinants

Required Qualifications:
  • Undergraduate degree in Social Work, psychology, or other health and human services degree.
  • 2+ years of experience in patient outreach
  • Working knowledge of medical terminology to communicate with members/beneficiaries and providers
  • Residence in Arizona

Preferred Qualifications:
  • Experience working with Centers for Medicare and Medicaid Services, especially Medicare and Medicare Advantage program
  • Solid communication and customer service skills
  • Experience working in electronic health system
  • Bilingual Spanish speaking
  • Knowledge of community resources
