The RN Diabetic Educator receives direction operationally from the Endocrinology RN Manager. Works collaboratively with Endocrinology Team focusing on meeting the educational needs of the type 1 and type 2 diabetic patient. Supports overall vision of C-I-Care patient experience by achieving a positive balance of health care, cost outcomes and patient satisfaction of diabetic patients. Promotes person-centered care, driving integration, a focus on behavioral health, leveraging technology, integrating related conditions and achieving the quadruple aim of population health, quality of the patients care experience, cost associated with care, and meaningfulness of the provider's experiences. Ensures standards of care are implemented and in compliance with all state and federal regulations, standards and practice and SMA Part of OptumCare policies and procedures.

Primary Responsibilities:
  • Responsible for the coordination of care for the diabetic patient referred from SMA Part of OptumCare Endocrinology Team. Develops / reviews individual needs assessment based on standards set by the American Association of Diabetes Educators (AADE)
  • Provides 1:1 visits to facilitate core concepts of diabetes self-management education per AADE
  • Reviews technology devices with diabetic pts (insulin pumps, continual glucose monitors, etc) and collaborates with ordering provider
  • Provides telephonic communication to support education process as needed
  • Utilizes approved clinical criteria to assess and determine appropriate level of care for the diabetic patient
  • Maintains Endocrinology AADE certification
  • Supports Endocrinology Department initiatives focused on the care and research of the diabetic patient

Required Qualifications:
  • RN License from the State of Nevada
  • Certified Diabetes Care and Education Specialists from the National Certification Board for Diabetes Educators (NCBDE)
  • 1+ years of clinical experience working with the diabetic population
  • Intermediate proficiency with Microsoft Office
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained

Preferred Qualification:
  • Bachelors' degree or higher in nursing, public health or other related field
