Venice Family Clinic is a nonprofit community health center celebrating 50 years of delivering quality primary care to people in need. We provide vital and comprehensive health care services to nearly 28,000 people each year, regardless of their income, insurance or immigration status. In this role, you will coordinate the delivery of health care services to patients in a primary health care clinic and provide administrative support to medical providers and medical staff. You will discharge patients after medical visits, arrange for follow-up appointments, process referrals for Medi-Cal, Medicare, Managed Care and County and you will request authorizations using the Web Portal and E-consult. You will also perform data collection and perform extensive patient tracking and follow-up care.

As our Clinic Coordinator, you will also review patient records and complete data collection forms using ICD9/ICD10 and CPT codes and consult with providers and conduct patient phone follow-ups. You will review health education material with patients as instructed by provider and will assist with training and supervision of volunteers.
Qualifications
  • Skill at coordinating the delivery of health care services to patients in a primary health care setting
  • Strong interpersonal skills to work effectively with people of diverse cultural, educational, economical, and social backgrounds
  • Strong written and verbal communication skills in both English and Spanish
  • Ability to prioritize and multi-task assignments with competing demands
  • Excellent organizational skills to complete work in a timely and efficient manner
  • Ability to discharge patients using NextGen Electronic Health Record system and follow up as necessary
  • Ability to triage medical problems
  • General knowledge of health education, public health, and medical terminology
  • Working knowledge of medical/social service agencies in the West Los Angeles area preferred
  • Ability to Multi-task, work independently or part of the coordinator team
  • Ability to work evenings as needed
  • Valid California driver's license and proof of auto insurance
