Case Management Coordinator - Central\/NW Kansas

Job Description
Qualifying candidates may be eligible for a sign on bonus of up to $3,000.

As a Case Management Coordinator you will utilize critical thinking and judgment to collaborate and inform the case management process with members. You will facilitate appropriate healthcare outcomes by providing care coordination, support and education for members through the use of care management tools and resources.

You will develop, implement, support, and promote Health Services strategies, tactics, policies, and programs that drive the delivery of quality healthcare to establish competitive business advantage for Aetna. Health Services strategies, policies, and programs are comprised of utilization management, quality management, network management and clinical coverage and policies.

This position will support Aetna Better Health of Kansas in the following counties: Norton, Phillips, Graham, Trego, Rooks, Smith, Ellis, Osborne, Russell, Rush, and Ness.

Daily local travel up to 25-50%.

Schedule: Monday-Friday standard business hours.

Required Qualifications
- 1 year experience in Home and Community based services (HCBS) waiver, behavioral health, social services, individuals with intellectual disabilities, severe emotional disturbance or appropriate related field equivalent to program focus
- Ability to complete daily local travel (25-50%)

Preferred Qualifications
- Case management and discharge planning experience
- Managed Care experience

Education
Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling).
