Position Summary:

The Care Management Assistant is responsible for performing administrative and operational functions needed in the Care Management Department. Oversees the administrative operation of the Care Management Department and acts as the administrative liaison between all clinical departments, community resources and third-party payers/insurance companies. The Care Management Assistant provides support to the Care Management staff in fulfilling the requirements of the Care Management Department.

Daily functions:
  • Organizes day to day departmental functions and acts as a care management departmental resource.
  • Efficiently assists with the daily operation of the Care Management Department
  • Performs data system entry and reporting daily


  • 1-3 years
  • Entry level knowledge; basic employment skills; High School diploma: GED equivalent or High School certificate
