The Clinical Program Consultant works in a leadership position as part of our Clinical Team. This individual works with UnitedHealthcare's contracted Community Based Partners, providing direct oversight and management to a diversified team of Geriatric Social Service Coordinators (GSSCs). This is a key role in developing solid relationships with our clinical partners and ensuring compliance metrics are met.

If you are located in Massachusetts, New Hampshire, or Rhode Island , you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:
  • Work closely with our Community ASAP (Aging Service Access Points) partners and GSSCs (Geriatric Service Support Coordinators) to implement and comply with our care model and visit requirements
  • Proactively monitors individual performances, ensuring performance metrics and goals are reviewed on a regular and frequent basis
  • Demonstrates a positive leadership shadow by shaping positive behaviors, influencing our values, and creating a healthy, high performance environment
  • Ensure compliance with Model of Care and State contractual requirements are met timely (chart audits), providing one to one coaching as needed
  • Demonstrates solid knowledge of computer functionality and software applications (e.g. navigating systems, accessing record management databases, accessing intranet, troubleshooting
  • Provide excellent customer service to our community partners
  • Work closely with sales and provider relations to promote and deliver our care model
  • Comply with our governmental contract requirements
  • Coordinate meetings at ASAPs, handle clinical issues and work closely with clinical region
  • Automobile travel to offices of our contracted providers and to meetings with the same in Massachusetts
  • Local travel around small geographic region

Required Qualifications:
  • Bachelor's Degree or higher
  • Unrestricted RN license or Licensed Social Worker
  • 3+ years of care management experience
  • 2+ years of clinical leadership experience
  • Intermediate proficiency in MS Office (Excel, PowerPoint, MS teams)
  • Reside in either Massachusetts, New Hampshire, or Rhode Island
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area


Preferred Qualifications:
  • Geriatric home care experience
  • Experience with Community ASAP partners and GSSCs (Geriatric Service Support Coordinators)
  • Knowledge of relevant state and federal guidelines (e.g., Medicare, Medicaid)
  • Proven interpersonal, communication and relationship building skills
  • Curious, adaptable, flexible, and able to work with changing direction and priorities
  • Ability to organize and thrive in a fast-paced matrixed environment
