If you are located NE Tennessee or Washington, Carter and Sullivan Counties, you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:
  • Develop Plan of Care
  • Provide patient education to assist with self-management
  • Interact with Medical Directors on challenging cases
  • Coordinate care for members
  • Make referrals to outside sources
  • Coordinate services as needed (home health, DME, etc.)
  • Educate members on disease processes
  • Encourage members to make healthy lifestyle changes
  • Document and track findings
  • Make the transition of care calls to ensure that discharged member receive the necessary services and resources
  • This is high volume, customer service environment. You'll need to be efficient, productive, and thorough in dealing with our members over the phone. Solid computer and software navigation skills are critical. You should also be have patient-focused and adaptable to changes
  • Work independently; work is frequently completed without established procedures
  • Work collaboratively with the Population Health team
  • Maintain calendars/coordinate meetings avoiding scheduling conflicts
  • Collaborate/facilitate community care team (CCT) rounds as needed (e.g., create PowerPoint slides; take notes)
  • Work cross-functionally and coordinate with others internally or externally to gather information to overcome challenges
  • This position may require providing coverage for internal team members as needed
  • Adhere to departmental policies and applicable internal and external/governmental guidelines and requirements (e.g., legal, regulatory compliance)
  • Educate and support non-clinical staff working with members care coordination
  • Perform other related functions as assigned

Required Qualifications:
  • Undergraduate degree or equivalent experience
  • Current unrestricted licensure in the state of Tennessee
  • Certified Case Manager (CCM) or the ability to obtain certification within 18 months of employment
  • Maintain CME/CEU's as applicable for licensure
  • RN or SW or Clinical Degree and 4+ years relevant work experience or undergraduate degree plus 4+ years Clinical Experience
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health or assisted living
  • Experience using Microsoft Office. i.e. Word (edit, create & save documents), Microsoft Excel (sorting, navigating & filtering data within spreadsheets), and Microsoft Outlook (email, folders, attachments, and calendar)
  • Must live in the state of Tennessee
  • Proficient (or higher) level of experience with Microsoft Office (PowerPoint, Word, Excel, & Outlook)
  • Proficient with relevant computer systems and platforms and ability to operate and integrate those systems (Intranet, Internet, SharePoint; ICUE, Microsoft Teams, Cisco WebEx, Zoom Meetings)
  • Ability to travel within 100 miles radius
