Community Health Worker

Job Description
Community Health Workers (CHW) serve the community as a bridge between our members and the healthcare system through outreach and education. They provide assistance to members with non-clinical social and health equity needs such as food, utilities and housing support in addition to benefit navigation. By providing these services, CHW's empower members to maintain better health outcomes and improve relationships with healthcare providers.

Required Qualifications
Must reside in Eastern KY area.

• 1-2 years' experience providing clinical services; experience in community/outpatient setting preferred.

• Experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred.

• Experience using a computer including the use of Excel and Electronic Health Records.

COVID-19 Vaccination Requirement

Preferred Qualifications
Community Health Worker Certification or equivalent training and experience preferred.

Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.

Experience working within the foster care and/or juvenile justice systems.

Education
• High School Diploma or G.E.D.

• Completion of approved CHW certification program within one year of hire date.
