Day - 08 Hour (United States of America)

The Program Coordinator within Stanford Health Care (SHC), Health Education, Engagement, and Promotion is responsible for a full range of planning, implementation and support of new and restructured programs that support Stanford Health Care (SHC) patient and community members. The Program Coordinator carries out assigned duties independently and communicates consistently with Management. The role is also in charge of planning and managing logistics for patients and program implementation. This role works collaboratively with a broad range of stakeholders in clinical, research, administrative, ancillary, and service departments, and manages communication among all departments and School of Medicine. The incumbent interfaces internally with SHC and School of Medicine staff and leaders across the Enterprise.

The Program Coordinator within HEEP for patient engagement and clinical integration will:
  • Outreach to patients and provide evidence-based interventions, such as behavioral therapy and pharmacotherapy counseling to patients whom are tobacco dependent.
  • Understands Care Coordination workflow and key components to help facilitate patient inquiries. Answers patients' questions related to their tobacco treatment over the phone or through the patient portal (MyHealth). Defers pharmacotherapy questions back to clinical care team as appropriate.
  • Coordinate all aspects of an individual's tobacco treatment care, including: providing evidence-based behavioral counseling via telemedicine and/or in-person based on patient preference; following up with patients to ensure cessation medications are received; referring individuals to additional supportive resources to achieve their tobacco cessation goals; using a CRM (Customer Relationship Management) to manage patient relationships and treatment plans; and charting interventions in Epic to communicate treatment plan back to the clinical care team.
  • Manages relationships with an interdisciplinary treatment team made up of providers, pharmacists, counselors, researchers, health educators and administrators.
  • Implement quality control measures to ensure internal operations and projects comply with department, hospital, and university policies.
  • Support projects to improve internal operations and overall growth; scope needs, develop plans, document and track project progress clearly and accurately.
  • Plan and implement logistics for (1) project communication on behalf of team and department, (2) videoconferencing with staff and/or patients, (3) project or department meetings.
  • Attend clinic huddles and meetings as appropriate to market programs, communicate progress, and gather input/feedback for improvement.
  • Handle the production of collateral marketing and event materials.
  • Communicates challenges and support needs to HEEP management and clinical care teams as appropriate.
  • Synthesize feedback from clinic staff and patients and present to team.
  • Assist with the selection process for consultants, contractors, and vendors.
  • Track, maintain, and analyze patient data to determine the appropriate interventions for unique patient populations and work with management to develop more effective referral pathways based on analysis findings.
  • Demonstrates ability to work with any customer, client, or vendor regardless of race, gender, sexual orientation, religious, or cultural beliefs
  • Acts as an ambassador for Stanford Health Care in dealing with the community in the course of carrying out all assignments.
  • Performs other related and incidental duties as needed or assigned.

Education Qualifications
  • Graduation from an accredited college or university wit h a bachelor's degree.

Experience Qualifications

Two (2) or more years related work experience in a healthcare or health related setting. Patient-facing and community-facing roles preferred.

Additional Preferred Qualifications
  • Tobacco Treatment Specialist (TTS) certification highly preferred.
  • Motivational Interviewing (MI) certification highly preferred.

  • Excellent written/oral/interpersonal communication skills. Ability to write clear, concise documents with attention to accuracy and detail

  • Ability to interact with diverse groups and an international audience.
  • Additional language skills (other than English) highly preferred.
  • Ability to develop materials for marketing, event planning, education, etc.
  • Ability to establish and sustain relationships with staff members and a variety of internal and external working groups and individuals.
  • Familiarity with or ability to quickly become familiar with the organization and array of services provided by an academic medical center.
  • Ability to maintain confidentiality, independent judgment and initiative required
  • Skilled and proficient in the use of computers (Word, Excel, PowerPoint, Canva, on-line databases, Zoom) and clinical applications (Epic)
