Population Health Business Consultant

Job Description
This is a telework role with standard business hours required.

Involved in initiative planning, ongoing client relationship management and satisfaction for assigned initiatives that impact the delivery of products and services to the internal and external environment (See Family Summary).

-Collaborates with external vendors, external consultants, consulting team members and/or matrixed staff as required in support of initiatives.
-Accountable for transferring knowledge, best practices (external and internal), methodology and tools to client organizations.
-Participates as a team member, may be responsible for concurrent initiatives or act as team lead for less complex projects based on resource requirements.
-Support the development of evidence based clinical programs and strategies that focus on improved health outcomes for the Medicaid population .
-Provides support to relevant departments and supports deployment of the programs.
-Support the development of clinical programs utilizing data analytics to support define performance metrics and improved outcome targets in both quality of care and financial domains.
-Ensures that standards of case management practice (i.e. behavioral, physical, and social determinants of health) and integrated services are developed that are consistent with professional standards, regulatory requirements and assures programs are compliant with corporate compliance agreement
-Develop and foster strong working relationships and communication at all levels of the organization, especially with departments with complimentary responsibilities.

Required Qualifications
An active and good standing RN, LPN, LCSW or LPC license required
- 3+ years of experience developing and creating new programs that are scalable
- 3+ years of experience leading projects
- 3+ years of experience implementing programs

-Demonstrated experience as a creative innovative contributor (thinking "outside of the box" is critical) in the development of new programs and ways of doing business.
-Demonstrated relationship management skills at the senior level; capacity to quickly build and maintain credible relationships at varying levels of the organization simultaneously.
-Demonstrated leadership with relevant initiatives: Business process, enterprise business project management/consulting, financial strategic planning and analysis
-Demonstrated experience successfully implementing change in complex organizations.

Bachelor's degree or equivalent experience
